Materials Use Request Form

You may apply for use of Archives materials by filling out the form below. Your request will be given to an archivist for review upon the receipt of the information.

If for any reason you have problems submitting your request via this form, you may email the requested information to archives@caltech.edu , or contact us by U.S. mail at the address found on the contact information page.

Description of your project
Please explain whether your use is personal or for research, display, web site, or publication. If for publication, please include full details, including author/producer, title of product, publisher and date of publication.
 
 
Materials requested
Please describe the materials requested as specifically as possible. For photos, specify ID numbers, format, size and finish (glossy or matte).  Note: The Archives does not guarantee the availability of all materials in its collections.
 
 
Rights requested
If you intend publication, please specify fully the rights you are requesting, for example: print, interior use; print, cover; TV; CD-ROM, etc., plus territory and time period.
